Provider Orders for Life Sustaining Treatment (POLST) program is designed to improve the quality of care at the end of life. It provides a standard form that can be used for patients diagnosed with serious illnesses throughout Minnesota. The POLST form identifies what types of medical treatment a patient wishes to receive at the end of life, and helps to ensure those wishes are conveyed to emergency services and other medical providers. The POLST form is used and recognized by hospital systems, long term care facilities, medical professionals, and emergency medical services throughout Minnesota. POLST is a medical document, not a legal document and does not replace a health care directive

Aging life care managers act as guides and advocates for older people and their families. Also known as senior care managers or geriatric care managers, these experts help older adults live at home rather than in a facility. They can provide any of the following services, although this is not an exhaustive list: * Assessment and monitoring * Planning and problem-solving * Match needs and services * Assist families with care coordination * Crisis intervention and updating levels of care as needs change These professionals typically have a background in social work, psychology, nursing, or gerontology. They also have specific expertise in assessing, planning, and serving the needs of older adults. The Five Wishes booklet is a person-centered advance care planning program. The workbook is a legal advance directive. * Five Wishes can be completed online or on paper. * Different booklets are for adults, young adults, adolescents and children. * Five Wishes meets the legal requirements for an advance directive in Minnesota. * The paper version is available in 32 languages.

Health Care Directive forms are available to download in Arabic, Chinese, English, Hmong, Russian, Somali, Spanish and Vietnamese. Education guides are available to help provide more information about the wishes outlined in the health care directive and terminology used. Certified advance care planning staff are available to provide information, documents, answer questions and can assist in properly completing your health care directive. Staff have scheduled times to be available at community locations in the Central MN area and by virtual zoom sessions to anyone in Minnesota.
