Resource Facilitation is a free, two-year statewide telephone support service for persons affected by brain injury, including Acquired Brain Injury (ABI) and Traumatic Brain Injury (TBI).
* This service offers support in transitioning back to family life, work, school and the community.
* The service is available to anyone at any point after a brain injury. Many participants are referred at the point of discharge from the hospital to support them in their transition.
Participant can receive scheduled calls at regular intervals for up to two years to ensure that their needs are met.
Types of services may include:
* Emotional support in coping with a new "normal"
* Finding mental health support and counselors
* Finding support groups
* Finding rehabilitation services, how to get assessed, primary care physicians
* Supporting caregivers with resources
