A Personal Care Assistant (PCA) is a trained worker who helps people that need support to take care of their day-to-day activities and who are on a state program (Medical Assistance, waiver programs, or Alternative Care). Personal assistance services help individuals live and work within their own communities. PCAs help people who need support to take care of daily living needs because of disease, a disabling condition, or complications related to aging. Personal care assistance services must be deemed medically necessary as determined by an assessment. The assessment must find that the person needs help to complete at least one of eight activities of daily living or exhibit Level 1 behavior. The four categories of services that a person can receive assistance in are: * Activities of Daily Living: eating, toileting, grooming, dressing, bathing, transferring (such as getting in and out of bed), mobility, and positioning * Health-related functions, which, under state law, can be delegated or assigned by a licensed health care professional to be performed by a PCA * Instrumental Activities of Daily Living: meal planning and preparation, managing finances, shopping for essential items, performing essential household chores, communication by telephone and other media, and participating in the community * Redirection and intervention for behavior including observation and monitoring If you are enrolled in Medical Assistance, a waiver program, or Alternative Care, there are two possible ways that you may receive PCA services in Minnesota: * You may select an individual worker (where you are responsible for hiring, supervising and firing) * You may hire a home care agency who selects and oversees your worker If you choose to hire your own workers, you will also choose a PCA Choice provider (usually a home health agency) as a fiscal intermediary. The PCA Choice provider is responsible for billing the Department of Human Services (DHS) for PCA services and paying your PCA workers. PCA has begun transitioning to Community First Services and Supports (CFSS). Individuals currently using PCA will transition to CFSS at their next reassessment.

A Consumer Directed Community Supports (CDCS) Support Planner helps people to write and use the person-centered Community Support Plan that is needed for CDCS. CDCS allows participants in waiver programs or Alternative Care to manage their own services and supports. These personalized supports help people to live at home and avoid or delay out-of-home placement. CDCS Support Planner services could include: * Providing information about CDCS, provider options and community resources * Helping to write a person-centered CDCS Community Support Plan * Helping to personalize a CDCS Community Support Plan based on the person's preferences and assessed needs * Helping the person to put their plan into action, and to monitor, evaluate and revise their plan * Helping to recruit, screen, hire, train, schedule and monitor workers The person and/or family caregiver decides how much help they need from the CDCS Support Planner.

This program provides support to people in abusive situations and works to reduce relationship abuse, intimate partner violence and family violence. Programs may include crisis intervention and domestic violence education and prevention. Financial management counselors take a look at your personal finance information and help to create a plan to pay off debt, keep on top of bills or provide retirement planning information. MORE INFORMATION Helps people to learn about finances and credit and provides counseling assistance for money management and budgeting. Financial counseling appointments are offered Monday - Friday. Financial Education Workshops: * Money Management * Consumer protection and identity theft * Living on reduced income during transitions Financial Literacy Classes: * Debt reduction and asset building * Consumer protection and financial institutions

MORE INFORMATION Offers meal delivery to people with: * HIV/AIDS * End stage renal disease * Chronic Obstructive Pulmonary Disease (COPD) * Cancer * Amyotrophic Lateral Sclerosis (ALS) * Multiple Sclerosis (MS) * Chronic heart failure Meal deliveries include a combination of fresh and frozen meals. Clients with children or caregivers in their home can also receive additional meals for their family members depending on funding. Menus offered for the following diets: * Medically tailored meals * Heart healthy * Kidney-friendly/Renal * Flavor neutral * Vegetarian * Gluten and dairy friendly (Not an allergen free facility) * Pureed diet The Cultural Meals Program includes culturally appropriate Hmong and East African clients.

They assess a client's needs, develop planned interventions, provide services to help reduce or stop the substance use, coordinate services with other providers, and reassess the client on a regular basis. This program is licensed for Substance Use Disorder Treatment by the Minnesota Department of Human Services (DHS) under MN statute 245G. Semi-Independent Living Services (SILS) help people to reach their goals and lead self-directed lives in the community. Services include training and assistance in independent living skills for adults with developmental disabilities or related conditions. Services provided may include training and assistance in: * First aid and getting help in an emergency * Learning and exercising rights and responsibilities of community living * Meal planning and preparation * Obtaining and maintaining a home * Personal appearance and hygiene * Self-administration of medication * Shopping * Social, recreation and transportation skills, including appropriate social behavior * Using the phone and other utilities * Managing money * Engaging in activities that make it possible for adults with developmental disabilities or related conditions to live in the community SILS staff may provide supported employment in community work environment settings for a recipient who already has a job but may be experiencing some challenges in maintaining it, or is at risk of losing such employment. This requires direct contact between the SILS recipient and staff person providing Support Employment. A SILS recipient may receive a one-time housing allowance of up to $1,500 to cover costs related to damage/security deposit for housing rental, utility deposit, household furnishings and other items necessary that will enable the person to secure a residence in which to receive SILS.
